The Expert Guide to Peru Travel Health: From Lima to the Andes

Updated February 2026

Whether you’re embarking on a high-altitude mountain bike tour or trekking the Inca Trail to Machu Picchu, Peru is a destination that demands high physical performance. The jump from the coastal air of Lima to the high elevations of Cusco and the Andes is the single biggest challenge for most travelers. My goal is to handle the clinical logistics so you can focus on the trail ahead.

As a Licensed Doctor of Pharmacy (PharmD) and faculty member at the University of Washington, I specialize in helping Washington adventurers prepare for high-altitude environments and the regional risks of South America.

Mastering the Altitude: The “PharmD Pro-Tip” on Diamox

Altitude sickness (AMS) is the primary concern for most Peru travelers. One of the biggest myths is that being in great physical shape protects you from the mountains—in reality, AMS can affect anyone regardless of fitness level.

  • The Dosing Detail: When discussing prevention with Acetazolamide (Diamox), more isn’t always better. While it is approved at higher doses, clinical experience shows that 125mg twice daily is just as effective as 250mg but with significantly fewer side effects (like that infamous fingertip tingling).

  • Beyond the Pill: We’ll look at your specific itinerary to see if you are flying directly into Cusco or navigating a route that involves significant elevation changes—such as biking through the mountains where you are constantly ascending and descending. We will build a proactive plan for hydration and medication timing so the shifting terrain doesn’t slow you down.

The “Amazon Insurance” Strategy

Peru offers incredible regional diversity, and many travelers want the flexibility to add an Amazon jungle tour (like Iquitos or Puerto Maldonado) at the last minute.

If there is a “maybe” on your itinerary for the jungle, my clinical recommendation is usually the “Better Safe” approach. Because vaccines like Yellow Fever require a 10-day lead time to be effective and are often a lifetime protection, I recommend getting travel vaccines before you leave. This “vaccine insurance” means you never have to say “no” to a once-in-a-lifetime adventure because you weren’t prepared.

The Peru Adventure Toolkit

Beyond the altitude, we’ll ensure your kit is ready for the rigors of the trail:

  • Typhoid & Gut Health: Whether it’s street food in Cusco or a meal at a remote campsite, we’ll plan for effective prevention and self-treatment of traveler’s diarrhea so you stay hydrated and energized.

  • Rabies Awareness: For mountain bikers and remote trekkers, we’ll discuss the reality of wildlife risks and whether pre-exposure prophylaxis is a smart addition to your kit based on your specific route.

Pharmacist’s Note: While the official CDC page for Peru provides a helpful baseline of requirements, my goal is to bridge the gap between those broad, generic recommendations and the reality of your trip. During our consultation, we translate that data into the context of your specific itinerary and real-world risk, ensuring your plan is as practical as it is protective.
